Job summary

Cynet HealthStaff is seeking an RN - Case Manager for a 12-week contract. The role is a 40-hour, Monday–Friday day shift in a hospital setting, with 7:00 AM to 3:30 PM or 8:00 AM to 4:30 PM scheduling based on unit needs.

The candidate must have 2 years of case management experience, be a licensed RN, and ACLS certification is required for most RNs. EPIC and Allscripts experience is preferred.

Qualifications

Responsibilities

• Conduct assessments on all admitted patients.
• Participate in interdisciplinary rounds.
• Initiate referrals for home health, durable medical equipment, SNF and LTACH as needed.
• Perform discharge planning and arrange for home health care.
• Discuss plans of care with patients and families when appropriate.
• Float within service line for high acuity discharges.
• Complete onboarding documentation and ensure compliance requirements.

Description:

Academic experience is preferred.

Duties involve discharge planning, assessment, arrangements for home health, and transitioning care outside the hospital.

The position may involve discussing plans of care with patients and families.

The role may include high acuity discharges.

Travelers must float within their service line.

The contractor must fulfill the terms of their signed agreement in full.

Additional documentation may be required as part of the onboarding process.

All certifications are due by the compliance deadline.

Modules are required to be completed prior to the start of the position.
