Job summary

Valor Healthcare is seeking a qualified Registered Nurse to provide Non-Clinical Case Management services for VTARNG service members in Colchester, VT. This contract role supports readiness and wellness by coordinating care, maintaining electronic patient profiles, and ensuring proper resource utilization.

The position requires an active Vermont RN license (or ability to obtain), 2+ years in case management, strong organizational skills, U.S.

Qualifications

• Active, unrestricted RN license (Vermont or ability to obtain).
• BSN preferred.
• Minimum 2 years of case management, care coordination, or utilization management experience.
• Strong organizational and documentation skills.
• U.S. Citizenship required.
• Ability to pass federal background screening.

Responsibilities

• Support Periodic Health Assessments and Soldier Readiness Processing events by providing case management services and coordinating care activities.
• Establish and maintain electronic case management profiles within required timelines following provider referral.
• Prepare and contribute to monthly case status reports by tracking Soldier care progression and readiness outcomes.
• Apply Individual Medical Readiness and deployment regulations while obtaining required medical release documentation.
• Maintain compliance with Health Insurance Portability and Accountability Act standards during all case management activities.
• Document medical and administrative information across required systems, ensuring accuracy and completion within established timelines.
• Prepare and review case files for Medical Evaluation Board and Physical Evaluation Board processes, ensuring completeness and accuracy.
• Communicate with Government personnel to track compliance and address case management requirements.
• Engage with Soldiers to explain requirements, obtain documentation, and ensure understanding of responsibilities and privacy rights.
• Manage medical profiles and contribute to reporting on profile status and updates.
• Identify and coordinate submission of medical documents in accordance with required processing timelines.
• Participate in monthly case reviews, presenting case information and supporting follow-up actions

Position Summary

The Vermont Army National Guard (VTARNG) Office of the Deputy State Surgeon has a requirement to provide Non Clinical Case Management Services for VTARNG service members (SM) thereby promoting fitness and personal wellness for operational readiness and deployability. Non-Clinical case Managers coordinate all aspects of the care of individual patients. They ensure proper utilization of services and resources and provide assistance within, between, and outside of VTARNG facilities

Schedule:

The contractor shall provide services between the hours of 0730-1630 on Monday through Friday. The contractor is required to support weekend soldier readiness events. This contract honors 11 paid federal holidays.

Location:

Performance shall be at VTARNG State Medical Detachment, Green Mountain Armory, Camp Johnson, Colchester, VT 05446.

Qualifications

• Ability to work independently in a structured federal environment

• Ability to pass federal background screening

Preferred Qualifications

• Experience supporting federal or military-affiliated programs
• Experience with medical readiness, occupational health, or wellness programs

This job is CONTINGENT upon contract award, client approval, completion of a favorable background investigation, and the ability to obtain and maintain a Public Trust / High Risk clearance.
