Job summary

Tennessee Quality Care Hospice in Camden is seeking an RN Case Manager to join their growing team in Camden, TN. This role bases out of Camden, covering Decatur and Benton counties with a Monday–Friday 8am–4:30pm schedule and occasional after-hours as needed.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation across the nursing process.
• Obtain data and assess information from patient’s physical, psychological, social, and spiritual factors affecting needs; initiate interventions with the care team.
• Initiate communication with attending physician and inter-disciplinary team to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, progress notes, reports and care plans timely.
• Supervise and document delivery of care by the Home Health Aide according to guidelines.

What You’ll Do

• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
