Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Camden. This field-based position manages a patient caseload, provides therapeutic nursing, coordinates care, and supports patients and families in their homes.

Territory includes Camden with coverage of Decatur and Benton counties. Schedule is Mon–Fri 8am–4:30pm, with occasional after-hours and weekend duties as needed.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess CPR and First Aid certification.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation.
• Gather data from patient physical, psychological, social and spiritual factors and coordinate with interdisciplinary team.
• Initiate communication with attending physician, team and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Support family and perform clinical pronouncement of death when required; maintain accurate records and care plans.

Territory: Based out of Camden covering Decatur and Benton counties.

Schedule: Monday thru Friday 8am-4:30pm. Occasional after hours and weekends as needed.

Qualifications:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid driver’s license and proof of insurance is required

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
