Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join our growing team in Camden. This field-based position manages a caseload of patients, coordinates care, and supports patients and their families in home settings.

The role requires collaborating with an interdisciplinary team, performing nursing assessments, planning and evaluating care, and ensuring timely documentation while maintaining compassionate, high-quality hospice care.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications are required.
• Knowledge of quality assessment and assurance procedures.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess information from patient physical, psychological, social, and spiritual factors impacting care and coordinate interventions with the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Maintain accurate patient records, charts, progress notes, and care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.

Job description

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Camden.

This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory: Based out of Camden covering Decatur and Benton counties.

Schedule: Monday thru Friday 8am-4:30pm. Occasional after hours and weekends as needed.

Qualifications:

• Graduate from an accredited registered nursing program

• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
