Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their team in Camden. This field-based role manages a patient and family caseload, providing therapeutic nursing and coordinating care in the home setting.

Territory covers Camden with Decatur and Benton counties. Schedule is Monday through Friday, 8:00am–4:30pm, with occasional after-hours and weekends as needed. Comprehensive benefits and opportunities for growth are offered.

Qualifications

• Graduate from an accredited registered nursing program.
• Valid RN license in state of practice.
• CPR and First Aid certifications are required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission, planning, implementation, and evaluation of nursing care.
• Gather data from patient and family to assess needs across physical, psychological, social, and spiritual factors.
• Coordinate care by initiating communication with the attending physician and interdisciplinary team.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and care plans in a timely manner.
• Supervise and document delivery of care by Home Health Aide per guidelines.

This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory:

Based out of Camden covering Decatur and Benton counties.

Schedule:

Monday thru Friday 8am-4:30pm. Occasional after hours and weekends as needed.

Qualifications:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid driver's license and proof of insurance is required

What You'll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
