Job summary

Cherry Hill Free Clinic in Michigan seeks a registered nurse to provide home-based care for patients recently discharged from hospital or rehab settings. You will collaborate with physicians and supervise LPNs, CNAs, and aides to deliver comprehensive care.

You will perform assessments, document progress, educate patients and families on plans, meds, therapies and safety, and coordinate discharges to achieve goals.

Qualifications

• Active, unrestricted RN license in the state.
• Minimum of one year of clinical nursing experience.
• Current CPR certification.
• Reliable transportation, valid driver’s license, and current insurance.
