Job summary

Your Health is expanding its mobile primary care team in the Blue Ridge area, inviting applications for a Visiting Nurse. This full-time role provides care directly in patients' homes and communities, offering flexibility beyond a traditional office setting.

Ideal candidates hold an unencumbered RN license in the state, have reliable transportation and a clean driving record, and bring experience in primary care, home health, or hospice.

Qualifications

• RN license unencumbered in the state you are applying
• Reliable transportation and valid driver's license, clean MVR
• Experience in primary care, home health, or hospice is preferred.

Responsibilities

• Provide Case Management to patients and families, including symptom management, medication management, and collaboration with the Interdisciplinary Group (IDG)
• Oversee patient plan of care with collaborating physician
• Provide supervisory visits and oversight for LPNs and CNAs
• Provide education to patients, families, and facility staff
• Provide oversight to Aides
