Job summary

Providence Care is seeking a full-time Registered Nurse to provide hospice case management, symptom control, and medication oversight for patients at home, in assisted living, and in long-term care facilities in the Gainesville area.

You will oversee the care plan with a collaborating physician, supervise LPNs and CNAs, and conduct start-of-care visits. Education and coordination with Care Navigators are essential to support patients and families during end-of-life care.

Qualifications

• RN license unencumbered in the state you are applying.
• Experience in Hospice/Home Health is preferred.
• Valid driver’s license and reliable transportation, clean MVR.

Responsibilities

• Provide Case Management to patients and families, including symptom and medication management.
• Oversee patient plan of care with collaborating physician.
• Provide supervisory visits and oversight for LPNs and CNAs.
• Conduct start of care visits and collaborate with Care Navigators.
• Educate patients, families, and facility staff.
• Oversee Home Health Aides and LPNs.
