Qualifications

Responsibilities

• Provide Case Management to patients and families, including symptom management and medication management
• Collaborate with IDG members to oversee patient plan of care
• Conduct start-of-care visits and supervise LPNs and CNAs
• Provide education to patients, families and facility staff
• Oversee Home Health Aides and LPNs
