Job summary

Bristol Hospice in Belfast, ME is seeking dedicated RN Case Managers to guide patients and families through the hospice journey. This full-time, day-shift role offers a competitive salary and a 60-mile coverage area around the Bangor office.

Schedule is Monday–Friday, 8am–5pm. You will develop care plans, monitor vitals, administer medications, and coordinate with interdisciplinary teams to deliver compassionate, high-quality end-of-life care.

Qualifications

  • RN license in good standing in the state of work.
  • Driver’s license and reliable transportation.
  • CPR certification (current).
  • Strong observational and communication skills.
  • Flexible, team-oriented attitude.

Responsibilities

  • Develop and implement individualized care plans based on medical history.
  • Deliver attentive, one-on-one support to patients and their families.
  • Conduct thorough assessments of physical, emotional, and spiritual needs.
  • Monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
  • Administer physician-ordered medications and treatments with skill and empathy.
  • Educate patients and their families on care techniques and symptom management.
  • Track and communicate changes in patient condition, ensuring timely and effective responses.
  • Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery.

What You'll Bring to The Team
  • A heart for hospice care and a commitment to exceptional service
  • A valid Registered Nurse license in good standing in the state you will work in
  • A valid driver’s license and reliable transportation
  • CPR certification (current)
  • Strong observational and communication skills—both written and verbal
