Job summary

Stony Brook Medicine is seeking a TH Staff Associate/Supervising Case Manager to provide clinical services and lead the Case Management team within Care Management. The role requires strong leadership, expertise in CM/Managed Care, and a focus on discharge planning to optimize patient flow.

Ideal candidates will have an RN license, a Bachelor's degree, five years of acute care experience, and PRI certification. Weekend/weekday flexibility is expected, with day and evening rotations as needed.

Qualifications

• RN license and a Bachelor's degree are required.
• Five years of acute care experience.
• Experience in Case Management or Managed Care.
• Working knowledge of observation, RAC, Cert, and MAC.
• Understanding of discharge planning and barriers to discharge.
• Experience with Unit Rounds and transitional care information.

Responsibilities

• Provide clinical services to patient population.
• Supervise Case Managers and Appeal Managers; oversee staff performance.
• Assist in budgeting, utilization review, staffing, and department systems.
• Mentor staff and coordinate department orientation for new staff.
• Deliver education and discharge planning training to interdisciplinary teams.
• Maintain high level of customer service and ensure essential coverage as needed.
