Job summary

Mount Sinai Morningside is seeking a skilled RN/Case Manager to oversee patient case management across units, ensuring appropriate level of care and timely plan implementation. You will collaborate with physicians, social workers, and home care teams to optimize LOS, streamline discharge planning, and improve resource utilization.

Required are a NY RN license and prior acute care/UR experience; CCM and PRI within 1 year are preferred.

Qualifications

• Bachelor's Degree in Nursing; Masters preferred.
• 3–5 years RN experience in acute care; home care/long-term care exposure preferred.
• Current NYS RN license. CCM certification preferred. PRI/PRI Certification to be obtained within 1 year. BLS issued by AHA.

Responsibilities

• Reviews all new admissions to identify utilization review and discharge planning needs using standardized criteria.
• Performs continued stay reviews to justify continued inpatient stay.
• Collaborates with physicians and clinicians to expedite testing, treatment and consultations.
• Documents all clinical reviews in the Case Management Documentation system.
• Assesses the patient’s clinical, psychosocial, and functional status with the interdisciplinary team.
• Identifies and documents variances affecting LOS and discharge planning; coordinates care transitions.
