Stony Brook University seeks a TH Staff Associate/Case Manager in the Care Management Department at Stony Brook Medicine. The role provides clinical services to patients, ensuring effective care coordination, appropriate levels of care, and collaboration with the care team to support excellent patient outcomes.

Qualified candidates will have a Bachelor's degree or be an RN working toward a degree with licensure, strong communication, and detailed documentation skills.

Qualificações

• Bachelor's degree or RN with license and 5 years hospital experience required.
• Good understanding of admission/discharge process; strong interpersonal skills.
• Accurate documentation and ability to prioritize work and follow up with cases.
• Preferred: CM, UM, Quality, Coding, Risk or MCG Certification; PRI Certification; regulatory knowledge; bilingualism.

Responsabilidades

• Completes Utilization Review for inpatient and observation cases with care team input.
• Reviews short stay, long stay, and complex cases to ensure safe discharge planning.
• Reviews Medicare cases for level of care on admission and coordinates with physicians for approvals.
• Communicates with transferring hospitals, Patient Access and payers for authorization prior to transfer.
• Maintains consistent EMR documentation on acuity, care plan, and inter-team communication.
• Participates in performance improvement activities and patient resource coordination.
