Job summary

Qualifications

• FL/compact RN license.
• BLS certification.
• 2+ years recent case management experience in an acute hospital setting.

POSITION SUMMARY – RN Case Management Lead

POSITION DUTIES

Oversees the case management team, coordinates patient care and discharge planning, ensures appropriate utilization of healthcare resources, collaborates with interdisciplinary teams, and monitors compliance with regulatory and quality standards to promote optimal patient outcomes.

Minimum Required Qualifications –

• FL/compact RN license
• BLS
• 2+ years recent case management experience in an acute hospital setting

Preferred Qualifications –

• Previous leadership or supervisory experience
• Acute rehabilitation hospital experience
• CCM / ACM

LENGTH OF ASSIGNMENT
