Qualifications

• Degree in Nursing; Bachelor's degree preferred.
• At least 2 years hospital patient care experience required.
• Home care, reimbursement, or hospital insurance experience preferred.
• Inpatient case management experience including utilization review recommended.

Responsibilities

• Coordinate care for assigned patients through collaboration with physicians, nursing staff, and other health professionals for efficient resource use and optimal care.
• Perform admission, continued stay, and discharge reviews to align with regulations and utilization plans.
• Collaborate with patient/family and interdisciplinary team to ensure care delivery across the continuum.
