Job summary

Baptist Health - Central Alabama is seeking an RN Case Manager to coordinate patient care across the continuum, including discharge planning and utilization management. You will collaborate with physicians, nursing staff, and other care professionals to optimize clinical resources and financial outcomes.

The role requires an RN license, a nursing degree, and at least 2 years of hospital patient care experience. Experience in reimbursement or hospital insurance is a plus.

Qualifications

• RN with nursing degree; Bachelor's degree preferred.
• At least 2 years hospital patient care experience required.
• Experience in home care, reimbursement, or hospital insurance preferred.
• Active RN license required; Case Manager certification preferred.

Responsibilities

• Coordinate care for assigned patients with physicians, nursing staff and other health professionals.
• Perform discharge planning and utilization management to ensure efficient care and financial outcomes.
• Complete admission, continued stay, and discharge reviews per regulations and policies.
• Collaborate with patient/family and interdisciplinary team; protect revenue and ensure proper bed billing.
