Under general supervision, the RN Case Manager is responsible for the ongoing development, implementation, and evaluation of processes of the Case Management Plan. This role requires a highly collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet clients' and patients' health and human service needs. Advocacy, communication, collaboration, resource management, and education to interdisciplinary team members are key responsibilities. Effective allocation of the hospital's resources while ensuring high-quality patient care is essential, as is adherence to regulations and policies relating to billing and fiscal intermediaries.
Responsibilities
- Maintain and improve the case management system through ongoing interaction with patients, physicians, and other health providers.
- Document case management plans in a clear and concise manner.
- Assist physicians, patients, families, staff, and payers in providing appropriate and cost-effective care for patient populations.
- Participate actively in departmental and interdepartmental meetings related to Case Management/Social Services functions.
- Collaborate with payers and review organizations to ensure proper utilization of patient care resources.
- Document all contacts and information concerning the patient's case management process appropriately.
- Educate physicians and staff regarding appropriate levels of care and utilization issues.
- Maintain current knowledge of case management/socio-services, discharge planning, and nursing issues through continuing education.
- Introduce new methods and techniques to enhance service delivery.
Required Experience / Certifications / Licensure
- Bachelor's Degree in Nursing (Master's degree preferred) with at least three years of clinical experience in a hospital setting.
- A minimum of five years of case management experience in a hospital setting.
- Certification in case management is preferred or a willingness to obtain within two years of employment.
- Current unrestricted Registered Nurse license in the State of Colorado is required.
- Current Basic Life Support (BLS) certification.
Schedule
This is a full-time position with weekend coverage of 5 hours for one Saturday each month; employees receive 5 hours off on one day of the week.
