Job summary

Alpha Home Health and Hospice in Missouri is seeking an RN for Medical Transition Management to support safe, seamless patient transitions across care settings within our Healthcare & Medical Services team.

You will coordinate discharge planning, perform medication reconciliation, arrange follow-up care, and educate patients and families on care plans and self-management, while collaborating with physicians and case managers.

Qualifications

• Current RN licensure in good standing.
• Strong clinical assessment and care coordination skills.
• Thorough knowledge of discharge processes and medication reconciliation.
• Effective communication, patient/family education, and teamwork.
• Ability to personalize care plans based on patient needs and access barriers.

Responsibilities

• Coordinate discharge planning and ensure continuity of care after transfer.
• Perform medication reconciliation to reduce errors and support safe use.
• Arrange and manage follow-up care to improve outcomes and lower readmissions.
• Educate patients and families on care plans, medications, and self-management.
• Collaborate with physicians, case managers, and support staff to personalize care.
• Identify and address barriers to access and adherence to the plan of care.
