Job summary

Molina Healthcare is seeking a Registered Nurse to support care transition activities and coordinate discharges from hospital to post-acute settings. The role focuses on reducing readmissions by ensuring safe transitions and accessing appropriate services with families and providers.

The candidate will conduct home visits, educate members on medications and follow-up care, and collaborate with an interdisciplinary team to optimize transition outcomes.

Qualifications

• Active RN license in state of practice.
• 2+ years in health care, with discharge planning or care management experience.
• Experience collaborating with hospital staff, providers and families.
• Proactive, detail-oriented and capable of independent work.

Responsibilities

• Support care transition activities from hospital to post-acute settings.
• Coordinate with discharge planners, hospitalists, providers and families.
• Ensure safe transitions with appropriate caregiving, meds, and oversight.
• Reassess needs using the Coleman Care Transition model post-discharge.
• Educate members on medication management, follow-up care and red flags.
• Facilitate interdisciplinary care team meetings and care coordination.
