Job summary

Option Care Health is seeking a Clinical Care Transition Specialist / RN in Cleveland to educate patients and families about home infusion services and coordinate care transitions from hospital to alternative sites. The role emphasizes building strong relationships with referral sources and partnering with account managers to grow referrals.

The ideal candidate is an RN (OH licensed) or a licensed Pharmacist with healthcare experience, capable of driving patient starts, conducting in-person

Qualifications

• Licensed RN or licensed Pharmacist in the state of practice with 2+ years in healthcare.
• Experience building relationships with internal/external stakeholders.
• Experience applying standards (ACHC, URAC) and ensuring quality of service.
• Proficient PC skills: internet, email, MS Office.

Responsibilities

• Proactively maintains and grows referral relationships to increase patient starts and sales.
• Educates patients, families, and facility staff about services for discharge to home or ATS.
• Coordinate care transitions with hospital staff, case managers, and internal partners.
• Ensure timely discharge, therapy independence, and proper orders transfer.
• Communicates with medical teams and caregivers throughout the discharge process.
