Job summary

Kaiser Permanente in Oregon seeks an Inpatient Care Manager (RN) to ensure appropriate level of care, coordinate post-discharge plans, and support physicians and the care team. This role emphasizes safe transitions, effective resource use, and high-quality patient care across acute and emergency settings.

The RN will independently assess needs, document plans, and advocate for patients while collaborating with interdisciplinary teams and maintaining regulatory compliance.

Qualifications

• Successful completion of an RN program by date of hire.
• High School Diploma or GED required.
• Credentials from multiple states; primary state at hire and additional states post hire.
• RN license in WA or compact license within 6 months; Oregon license within 6 months.
• Basic Life Support within 3 months of hire.

Responsibilities

• Coordinates post-discharge patient care to assure timely discharge.
• Independently completes patient assessments that are thorough and age-appropriate.
• Develops plans of care with patients and families to optimize resources.
• Facilitates safe transfers and ensures appropriate care at the right time.
• Maintains ongoing contact with interdisciplinary staff for safe transitions.
• Ensures regulatory and compliance standards are met.
• Performs duties as requested.
