Kaiser Permanente is seeking an Inpatient Care Manager (RN) in Oregon to independently assess patient needs, coordinate post-discharge plans, and ensure appropriate levels of care across acute and transition settings. You will collaborate with physicians, nurses, and care teams to optimize resources while maintaining high-quality patient care.

The role requires at least two years of RN experience in acute or transition care areas, multi-state licensure by hire, and certification in basic life

Qualifikationen

• Minimum two years RN experience in Med/Surg, ICU, ED, Home Health, SNF, Hospice or similar.
• Successful completion of RN program; high school/GED required.
• Credentials from multiple states with primary state license at hire; secondary state credentials post hire.
• Basic Life Support within 3 months of hire.
• Ability to coordinate care and discharge planning with diverse teams.

Aufgaben

• Coordinate post-discharge care to ensure timely, safe discharge for routine and complex patients.
• Conduct comprehensive patient assessments, documenting needs, benefits, and level-of-care determinations.
• Develop and communicate plans of care with patients and families; optimize resource use.
• Facilitate safe transfers and transitions to appropriate care settings.
• Ensure regulatory and compliance standards are met in collaboration with the care team.
• Provide expert education and support to physicians and staff on transition care.
