Job summary

One Community Health (OCH) is seeking a Registered Nurse (RN) Care Team Case Manager to support medical providers and the clinical care team. The RN CTCM will deliver care coordination, case management, and acute interventions following established protocols to promote high-quality, patient-centered outcomes.

This full-time role is located at our Stevenson, Washington clinic with weekday shifts of 8 or 10 hours.

Qualifications

• Graduate of an accredited nursing program.
• Current Washington State Registered Nurse license.
• Minimum two years of RN experience; ER, triage, or care coordination/case management preferred.
• Experience in adult and pediatric primary care.
• Current CPR/BLS certification.
• Knowledge of HIPAA regulations and patient privacy.
• Strong communication with clinical and non-clinical audiences.
• Proficiency with Microsoft Office and Epic.
• Ability to manage multiple deadlines and priorities.
• Strong safety and customer service practices.

Responsibilities

• Provide care coordination and case management support for patients.
• Deliver acute interventions per established protocols.
• Support transitions of care and care continuum activities.
• Communicate clinical concepts effectively to clinical and non-clinical audiences.
• Operate medical equipment and skills such as venipuncture and IV procedures.
