Miami Valley Hospital North is seeking a Case Manager (RN) to assess patients, coordinate care, and plan transitions to maximize outcomes and appropriate length of stay. The role requires collaboration with an Integrated Care Management Program and coordination with physicians, payors, and families.

The ideal candidate holds a BSN, Ohio RN license, and 3+ years of relevant clinical experience, with preferred certifications in case management or critical care.

Formación

• BSN required from an accredited nursing program.
• Ohio RN license; specialty certification preferred.
• Minimum of 3 years recent clinical experience in the patient population.
• Experience with clinical documentation management and discharge planning preferred.
• Familiarity with InterQual ISD-A and Milliman Guidelines is preferred.
• Proficient in Microsoft Office.

Responsabilidades
