Mount Carmel Health System is seeking an RN Care Navigator to coordinate and deliver patient care within an ambulatory setting in line with the Ohio Nurse Practice Act. You will operate within a collaborative team, using the Nursing Process to ensure high-quality patient outcomes.

• RN Care Navigator role requires coordination of care using the Nursing Process.

• Out reaching to patients.
• Completing visits with patients.
• Escalating concerns to providers as needed.
• Communication, education and scheduling of appointments.
• Utilizes EPIC for documentation and workflow.
