The Ohio State University James Cancer Hospital is seeking a Nurse Care Management - Outpatient Oncology Patient Care Resource Manager to guide patients through the care continuum, from pre-admission to post-discharge coordination.

You will partner with physicians, nurses and care teams to optimize throughput, apply the nursing process, and remove barriers to timely decision making while ensuring high-quality, cost-efficient care.

Formación

• Bachelor's degree required; Master's preferred.
• RN licensure required; five years clinical experience preferred.
• Knowledge of clinical practice guidelines, computer databases, and payer protocols.

Responsabilidades

• Coordinate pre-admission through post-discharge care using nursing process.
• Collaborate with physicians, nurses and other health care workers to manage transitions.
• Identify and remove barriers to progression of the plan of care for each patient.
• Assist physicians in ensuring appropriate level of care using evidence-based criteria.
