Job summary

Mount Carmel Health System in Ohio is seeking an RN Care Navigator to coordinate and deliver patient care within the Ohio Nurse Practice Act. The role supports primary care across Mount Carmel Medical Group locations with standard weekday hours.

The ideal candidate holds an RN license, an associate degree (BSN preferred), and at least 2 years in ambulatory care with care management experience encouraged.

Qualifications

• Graduate of a school of nursing.
• Active, unrestricted Ohio RN license.
• Minimum 2 years ambulatory nursing experience preferred.
• 1 year care management experience preferred.

Responsibilities

• Conduct Medicare annual wellness visits.
• Outreach to patients and coordinate visits.
• Complete visits with patients.
• Escalate concerns to providers as needed.
• Coordinate communication, education, and scheduling of appointments.
• Utilize EPIC system for documentation.

Position Purpose

RN Care Navigator, within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. The Registered Nurse Care Navigator, functioning in the spirit of teamwork, is accountable for the coordination and delivery of patient care utilizing the Nursing Process within the standards, policies, procedures and guidelines of the Organization.

Minimum Qualifications

• Education: Graduate of a school of nursing. Associate's Degree required Bachelor's preferred.
• Licensure / Certification: Registered Nurse; current, unrestricted license to practice in the State of Ohio.
• Experience: Minimum of two years experience in an ambulatory setting preferred.
• One year Case / Care Management experience preferred.
