The Clinical Navigator is a Registered Nurse who specializes in guiding patients and their families through the complexities of the healthcare system, particularly during major health events such as a new diagnosis, hospitalization, or a complex treatment plan. They act as a central liaison between patients, medical providers, and community resources to reduce barriers to care, improve patient outcomes, and ensure a seamless care experience. As a Clinical Navigator, you will serve as the primary point of contact for business development, referral management, and community outreach. You will represent our LTACH Specialty Hospital services to facility staff, attending physicians, hospitals, patients, and families, ensuring they understand our compassionate approach to end-of-life care. This role will include public relations, education, and relationship-building between the Landmark Hospital team and the communities we serve.
