Qualifications

• Bachelor's Degree in Nursing is required.
• 5 years of experience as a registered nurse (RN).
• 2 years of experience in case management, disease management, home health care, hospital, or physician office setting.

Responsibilities

• Identify patients falling short of clinical goals and arrange follow-up per protocol.
• Update medication lists and document allergies in the EHR.
• Use data registries and evidence to support point-of-care decisions and quality improvements.
• Establish external relationships with hospitals and post-acute facilities.
• Provide care navigation services and coordinate with LVHN Specialty Institutes and care programs.
