Job summary

Wollborg Michelson Recruiting seeks an experienced Registered Nurse to provide in-person and virtual support for specialist and diagnostic services. You will educate patients, coordinate referrals, schedule appointments, and follow up to improve outcomes.

The role requires a valid RN license, CPR certification, and EMR experience. You will engage with care teams and community partners while ensuring confidentiality and quality care across multiple channels.

Qualifications

• Active, unrestricted Registered Nurse license.
• Current CPR certification.
• Experience with electronic medical records.
• Experience supporting patients, coordinating referrals, and managing clinical services.
• Reliable transportation and a valid driver's license.

Responsibilities

• Provide in-person and virtual support through real-time consultation, patient education, referral coordination, appointment scheduling, and follow-up for specialist and diagnostic services.
• Support improved patient health outcomes and cost savings.
• Explain specialist and diagnostic service recommendations and referral requirements.
• Manage the referral and specialist service process from beginning to end.
• Lead quality programs, including patient outreach related to care gaps and clinical needs.
• Support patients with chronic conditions through education, reminders, and ongoing assistance.
• Submit service and appointment requests on behalf of patients.
• Provide the care team with referral information and requirements.
• Build relationships with healthcare providers and community organizations.
• Perform other assigned duties.
