Job summary

Sentara is seeking an experienced RN Navigator to join our team at Obici Hospital. The role supports all of Hampton Roads and surrounding regions, with a homebase at Obici Hospital and a remote-work option.

You will coordinate care from screening through follow-up, develop comprehensive care plans, and work closely with clinicians to ensure smooth transitions and quality outcomes for patients and families.

Qualifications

• Nursing Degree Required - BSN, ADN, or Diploma.

Responsibilities

• Facilitate comprehensive care coordination across the continuum.
• Develop and implement aCare plans based on assessment of patient needs.
