Job summary

Sentara Health Plans in Carrollton, VA is seeking an experienced RN Navigator for the CT Lung Screening program. This role supports patients from order to completion with remote work flexibility and a 7:30am–4:00pm schedule.

You will coordinate care, schedule screenings, and collaborate with the care team to ensure smooth transitions. Requires a nursing degree, Virginia RN license, BLS, and 3 years RN experience; oncology/pulmonary preferred.

Qualifications

• Nursing degree required (BSN, ADN, or Diploma).
• Virginia RN license (multi-state) required.
• BLS certification within 90 days of hire and maintained thereafter.
• Minimum 3 years RN experience.
• Oncology, pulmonary, or thoracic experience preferred.
• Specialty case management certifications (ACM, CCM, CCCTM, CMAC or CGMT-BC) preferred.

Responsibilities

• Coordinate and facilitate comprehensive care across the continuum.
• Develop and implement care plans based on assessments.
• Collaborate with healthcare team and patient/caregiver to manage care and transitions.
• Support navigation from order to completion for CT lung screening.
