Sentara is seeking an experienced RN Navigator to join our team at Obici Hospital. The role supports all of Hampton Roads and surrounding regions, with a homebase at Obici Hospital and a remote-work option.

You will coordinate care from screening through follow-up, develop comprehensive care plans, and work closely with clinicians to ensure smooth transitions and quality outcomes for patients and families.

City/StateCarrollton, VAWork ShiftFirst (Days)Overview:We are looking for an experienced RN Navigator to join our team!Hours: Monday - Friday - 7:30am - 4:00pmLocation - Homebase Obici Hospital - ability to work remotely - Supports all of Hampton Roads, Northern Virginia, and Northeast North Carolina.The CT Lung Screening Navigator supports patients from order to completion. They look at diagnostic codes and determine if they meet criteria for the screening, help get them scheduled, and support navigation during follow up care. We are looking for an RN who is detail oriented, a self-started with good follow through.Job Description:Responsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs. Functions in one of the following practice settings: Acute Care, Service Lines, Ambulatory/Community-based, Home Health, and Long Term Care.Requirements:Nursing Degree Required - BSN, ADN, or DiplomaRN License is required in Virginia Multi-State.BLS required within 90 days of hire and maintained thereafter (if in clinical setting)3 years of RN experience required. Preference is experience in oncology, pulmonary, or thoracics.Specialty certification in case management or related area highly preferred. (ACM, CCM, CCCTM, CMAC or CGMT-BC)3 years Case Management experience preferred.We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $89,544.00 - $136,468.80 annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
