Job summary

Kaiser Permanente is seeking an Inpatient Care Manager RN to ensure patients are admitted to the correct level of care and to coordinate transitions across acute and post-acute settings for optimal reimbursement and quality care.

You will assess needs, develop plans of care, educate physicians and staff, and support discharge planning while maintaining safe, cost-effective care. Strong communication and teamwork are essential.

Qualifications

• RN license with eligibility in WA or OR.
• Minimum two years RN experience in Med/Surg, ICU, ED, Home Health, SNF, Hospice, LTAC or Inpatient Rehab.
• Successful completion of RN program by date of hire.
• Basic Life Support (BLS) certification.

Responsibilities

• Coordinate post-discharge care to ensure timely and safe discharge from hospital.
• Perform and document patient assessments with age-appropriate, psychosocial considerations.
• Collaborate with patients, families and care team to develop care plans.
• Facilitate safe transitions to lower levels of care and optimize resource use.
• Maintain ongoing interdisciplinary communication for safe transitions across settings.
• Ensure regulatory and compliance standards are met in collaboration with the team.
• Perform duties as requested.
