Job summary

Bristol Bay Area Health Corporation in Alaska seeks an experienced Registered Nurse Case Manager to initiate and coordinate comprehensive nursing care for a diverse population ranging from neonates to geriatrics. The RN Case Manager will work as a vital member of the multidisciplinary team to improve outcomes and ensure timely, accessible care in a rural setting.

Responsibilities include managing acute and chronic care needs, educating and advocating for patients, and maintaining accurate EHR

Qualifications

• Active and unrestricted RN License required; Alaska licensure within 1 year of hire.
• Basic Life Support, Advanced Cardiac Life Support, and Pediatric Advanced Life Support certifications required.
• Minimum of two years' clinical experience; exposure to multiple units preferred.
• Ability to work across departments with diverse patient populations (acute, chronic, neonate, pediatric, geriatric).
• Proficiency with EHR, with emphasis on accuracy and compliance.
• Ability to communicate effectively with patients, families, and interdisciplinary teams in a culturally sensitive manner.
• Familiarity with rural healthcare challenges and limited access to specialty services.

Responsibilities

• Demonstrates knowledge of unit-specific protocols, workflows, and patient care standards.
• Manages a panel of patients' acute and chronic care needs with education and advocacy.
• Delivers safe, evidence-based nursing care across hospital units and settings.
• Assesses, prioritizes, and initiates care coordination based on patient needs.
• Collaborates with interdisciplinary teams to ensure coordinated, high-quality patient care.
• Maintains accurate and compliant electronic health records, meeting documentation standards.
