Job summary

Mercy Cedar Rapids is seeking a Patient Care Coordinator to support patient-centered care. You will oversee complex nursing cases, coordinate discharge planning, and collaborate with physicians, departments, and payors to ensure timely, quality care.

The role requires an RN license in Iowa, a baccalaureate degree in nursing, and BLS certification, with experience in case management and discharge planning preferred. This is an on-site, part-time position with Mercy’s Cedar Rapids facility.

Qualifications

• Bachelor’s degree in nursing required.
• Current RN license in Iowa.
• BLS certification required per AHA.
• Experience in case management and discharge planning preferred.

Responsibilities

• Complex care review of patient status.
• Collaborates with other programs and departments to coordinate care.
• Develops understanding of referral sources for caseload.
• Acts as liaison between physicians and departments for timely care delivery.
• Assists in teaching patients and families as needed.
• Performs peer-to-peer reviews with payors to ensure reimbursement.
• Communicates plan of care with physicians and staff; coordinates care conferences.
• Applies InterQual criteria for continued stay reviews.
