Job summary

MercyOne Medical Group in Des Moines, IA seeks a Nurse Senior – Complex Care Specialist to coordinate care for acutely hospitalized patients with high complexity. The role operates within the Acute Care Management team and focuses on discharge planning and seamless handoffs to outpatient resources.

The ideal candidate holds a Bachelor’s in Nursing with at least three years of experience in complex inpatient care and Iowa RN licensure.

Qualifications

• Bachelor’s degree in Nursing or related field.
• Three (3) years of experience working with complex, high-risk patient populations in an acute or inpatient setting.
• Current Iowa RN license (MercyOne Genesis, excluding Aledo and DeWitt) or license from Illinois with a pending transfer within 12 months.
• Mandatory Reporter training within three months of hire.

Responsibilities

• Leads interdisciplinary complex care conferences to align patients and teams around the inpatient care plan.
• Coordinates complex inpatient care through discharge with timely connection and clear handoff to post-acute resources.
• Identifies and addresses system barriers to ensure safe, efficient progression of care.
