RiverView Health in Crookston, MN seeks a Clinical RN – Care Coordinator (Full-Time) to manage care for adults, children and families with complex medical and behavioral health needs. This role emphasizes care coordination, access to services, and education to improve outcomes.

The RN will develop individualized plans, support chronic disease management, and collaborate with patients, families, providers, and community partners to achieve healthcare goals.

Formación

• Current Minnesota RN license is required.
• Current American Heart BLS certification is required; training must be obtained within 4 months of hire.
• Current American Heart ACLS certification is required; training will be provided within the first year.

Responsabilidades

• Coordinate care for high‑risk patients with complex healthcare needs.
• Develop, implement, and monitor individualized patient care plans.
• Complete patient screenings and assessments to identify healthcare and service needs.
• Coordinate medical, behavioral health, dental, crisis, and community‑based services.
• Provide patient and family education and support chronic disease management.
• Connect patients with community resources and follow up to ensure service utilization.
• Communicate progress and barriers to care with patients, families, and providers.
• Maintain accurate and timely care‑management documentation.
