Job summary

Riverland Community Health in St. Paul, MN seeks a Community Healthcare Worker to collaborate with clinical care teams and mental health professionals to connect patients with community resources and support goal setting, monitoring, and improving patient experience and outcomes.

You will engage in disease management coaching, help enroll patients in housing, food, financial programs, assist with appointments and follow-ups, and document in the EMR while coordinating care across providers.

Qualifications

• Completion of GED or high school diploma required.
• Bachelor’s degree in a health-related field preferred.
• One year of community-based advocacy and support with significant public contact required.
• One year of community healthcare work in a medical clinic preferred.

Responsibilities

• Connect patients to community services and resources.
• Assist patients with understanding health management plans and goals.
• Coach patients in managing chronic conditions and self-care.
• Assist with housing, financial, food, and other essential resources.
• Enroll patients into programs and coordinate follow-up referrals.
