Job summary

Riverwood Healthcare Center is seeking a Registered Nurse (RN) to serve as Behavior Health Coordinator in Aitkin, MN. This full-time role provides patient-centered care through a collaborative, team-based approach, focusing on prevention, chronic disease management and care coordination within a population health framework.

The RN collaborates with providers and a multidisciplinary team to deliver high-quality outcomes, mentors staff, and champions evidence-based practice across care settings.

Qualifications

• Current, unrestricted RN licensure in Minnesota is required.
• BLS certification required.
• ACLS certification required or willingness to obtain within 90 days of hire.
• Experience in primary care, ambulatory care, population health or care coordination preferred.
• Proficiency with electronic health records; Epic experience preferred.

Responsibilities

• Provide patient-centered nursing care across the continuum of care.
• Collaborate with providers and interdisciplinary team members.
• Serve as a clinical resource, mentor new employees and advocate for patients.
• Support quality outcomes through evidence-based practice.
• Coordinate care activities such as CCM, TCM, BHI, PCM, ACP as needed.
