RiverView Health in Crookston, MN seeks a Clinical RN – Care Coordinator to manage care coordination for adults, children and families. This full-time role focuses on access to services, chronic disease management, and collaboration with teams and community partners.

The position requires Minnesota RN licensure, BLS and ACLS, with optional BSN. Weekend rotation and comprehensive benefits are included in a supportive, professional setting.

Formación

• Current State of Minnesota RN license is required.
• BLS certification required; ACLS certification required; training provided if needed.

Responsabilidades

• Coordinate care for high-risk patients with complex healthcare needs.
• Develop, implement, and monitor individualized patient care plans.
• Complete patient screenings and assessments to identify healthcare and service needs.
• Coordinate medical, behavioral health, dental, crisis, and community-based services.
• Provide patient and family education and support chronic disease management.
• Connect patients with community resources and follow up to ensure service use.
• Communicate with patients, families, providers, and partners on progress and barriers.
• Maintain accurate care-management documentation.
