Job summary

AdventHealth Orlando is seeking an experienced Registered Nurse for care management and discharge planning. The role focuses on coordinating patient care across the multidisciplinary team to ensure timely discharge and involve families in planning and decision-making.

You will assess patients, collaborate with payors for post-acute authorization, and coordinate community resources to support safe transitions from hospital to home or facilities.

Qualifications

• RN with active license required
• Experience in case management/utilization management preferred
• Knowledge of CMS Conditions of Participation for Discharge Planning
• Ability to educate patients and families
• Strong communication with payors for post-acute authorization
• Proficiency with electronic medical records systems
• Ability to work in a fast-paced hospital environment

Responsibilities

• Participates in multi-disciplinary rounds to review patient status and discharge plans
• Identifies resources for discharge and ensures timely transition
• Educates patients and families on health, social and financial aspects
• Assesses readmissions and escalates care delays to leadership
• Documents discharge planning evaluations and plans in medical records
• Coordinates post-acute care services and community resources
