Qualifications

Responsibilities

• Monitors the recovery of discharged patients through the Patient Call Back Program.
• Calls discharged patients to ensure satisfaction and compliance.
• Assesses patient knowledge and understanding of discharge instructions.
• Identifies gaps in the discharge treatment plan.
• Provides patient education as planned by the discharge team.
