Job summary

AdventHealth in Ocala, FL is hiring a full-time RN for Case Management and discharge planning. The role focuses on advocating for patients and families, coordinating post-acute care, and ensuring timely transitions. Schedule is day shift with Mon-Fri and rotating weekends.

Sign-on bonus and relocation incentives are available for eligible candidates. The position requires an Associate's Degree in Nursing and an active RN license, with acute care and case management experience preferred.

Qualifications

• RN acute care experience required.
• RN Case Management experience preferred.
• Associate's degree in Nursing required.
• RN license/certification requirements per site policies.

Responsibilities

• Provides patient and family advocacy, and support patient’s choice and patient rights during hospitalization.
• Communicates with payors patient’s needs for authorization for post-acute care as needed.
• Assesses patients’ and families’ wholistically for discharge planning needs in inpatient, observation and/or ED settings.
• Reviews the medical record and incorporates clinical, social, and financial factors into transition of care plan.
• Develops discharge plans with contingency plans to ensure timely care coordination.
• Leverages technology and follows standard work to ensure continuity of care and accurate discharge reconciliation.
• Participates in multi-disciplinary rounds to review patient status and resource needs for discharge.
• Educates patients and families regarding emotional, social, and financial impacts of illness and accesses community resources.
