Job summary

Provider Partners Health Plan in Greenfield, IN is seeking a Nurse Care Coordinator to execute the Model of Care through assessment, planning, and care coordination for PPHP members. You will advocate for services, collaborate with the care team, and promote high-quality, cost-effective outcomes for residents with complex chronic conditions.

The role requires current RN licensure, strong case management skills, and experience with Medicare/Medicaid; travel and driver’s license are often needed

Qualifications

• Current unrestricted RN license.
• Minimum of 3 years clinical care experience.
• Case management experience preferred.
• Knowledge of Medicare/Medicaid.
• Ability to communicate with interdisciplinary team.
• Proficient in nursing/medical databases.

Responsibilities

• Case identification/risk stratification to assess needs.
• Collaborate with Transitional Care Team to reduce readmissions.
• Conduct comprehensive assessment of barriers to care.
• Coordinate integrated care plan with nursing and interdisciplinary team.
• Facilitate referrals/consultations.
• Document care plans in the EMR.
• Manage outcomes and evaluate achievement of goals.
• Ensure Model of Care training for staff and contractors.
