Job summary

Palm Primary Care is seeking an experienced RN Clinical Coordinator with leadership to join our team in Texas. The role coordinates plans of care for hospital and nursing home patients from admission to discharge to ensure continuity of care.

Responsibilities include admitting clients, coordinating care, coordinating transfers, monitoring LOS, and leading weekly interdisciplinary meetings while maintaining accurate records and communicating with managers. Hybrid work at three locations.

Qualifications

• Active TX RN license and at least 3 years of RN Clinical Coordinator experience.
• Solid knowledge of case management principles, management and reimbursement.
• Effective communication and organizational skills.
• Bilingual preferred.

Responsibilities

• Admit new clients and conduct orientations.
• Coordinate care from admission to discharge and ensure continuity of care.
• Monitor cases, document progress and advocate for services.
• Lead weekly interdisciplinary meetings and communicate findings to the team.
• Coordinate transfers and placement in facilities as directed by Medical Director or PCP.
