Job summary

Socket.dev's healthcare division in Lansing, MI seeks an experienced Case Manager to coordinate patient transitions and ensure appropriate care levels across the continuum. You will collaborate with physicians, nursing, social work, and post-acute partners to improve outcomes and resource use.

Responsibilities include initial assessments, discharge planning, referrals to social services and DME, and daily liaison with the care team.

Qualifications

• State licensure as a Registered Nurse (RN) is required.
• Bachelor’s degree in nursing or actively pursuing degree and to be obtained within five years of accepting position.
• Three years of acute hospital care experience.

Responsibilities

• Coordinate care transitions to appropriate levels of care.
• Collaborate with social worker and other disciplines for discharge planning.
• Assess patient/family needs to reduce barriers and plan discharges.
• Communicate with physicians, patients, families, and care teams.
• Document assessments, plans, and interventions in EMR to facilitate discharges.
