Job summary

Plymouth Harbor is seeking a dedicated Registered Nurse to serve as liaison and advocate for resident clients in a 40-hour week with on-call rotation. Primary responsibilities include managing resident care, coordinating with physicians and families, and ensuring adherence to care plans and Plymouth Harbor policies.

The role requires Florida RN licensure, strong communication skills, and proficiency in MS Office.

Qualifications

• Must be a Registered Nurse in Florida and meet the criteria in Florida Statutes.
• Preferred 3–5 years experience in case management, home health care, or hospital nursing.
• Strong communication and documentation skills.
• Fluency in English.
• Proficiency with Microsoft Office programs.

Responsibilities

• Manage all aspects of resident client care needs.
• Communicate with residents, physicians, families, and care partners.
• Attend physician appointments with residents.
• Review health insurance policies and other non-financial information for residents.
• Provide crisis intervention (ER visits, hospital stays).
• Advocate for resident clients.
• Coordinate and supervise supportive services.
• Ensure professional nursing standards are maintained.
• Follow Plymouth Harbor procedures and resident care policy.
• Write and execute individual care plans for residents.
• Provide progress reports to physicians and families.
• Maintain resident confidentiality.
• Ensure care management aligns with plan of care and medical regime.
• Coordinate and document all nursing services.
