Job summary

Careforth is seeking an RN Care Manager to provide personalized care and support to caregivers and their loved ones in Rhode Island. You will conduct home visits, develop care plans, and coach families using our app. No night shifts or weekends required, with a flexible schedule and remote-first culture.

Active RN license in the state, 2+ years of elder or disability care, and ability to travel within Rhode Island are required. The role offers a pay range of $75,000–$90,000 annually.

Qualifications

• Active RN license in the state of employment.
• Minimum two years of direct elder or disability care experience.
• Ability to travel within Rhode Island; valid driver’s license and auto insurance.
• Experience in case management, care planning and assessment in health care settings.
• Proficiency in common software and web-based applications (MS Office, care management systems).
• Additional state-specific qualifications may be required.

Responsibilities

• Conduct home visits in assigned territory to assess home safety and health status.
• Provide ongoing coaching and support to caregivers via app, including resource referral.
• Develop trusting relationships with caregivers to influence change and reduce burnout.
• Document and review records for accuracy and regulatory compliance.

Position Summary

Our care management team is seeking an RN Care Manager to provide personalized care and support to caregivers and their loved ones in and around the surrounding areas of Westerly, North Kingstown, West Kingstown, Wakefield, and West Greenwich, Rhode Island. There are No night shifts or weekends required, and no on-call duties! As an RN Care Manager, you'll assess needs, create individualized care plans, conduct home visits, and ensure exceptional consumer care. What You Will Do

• Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support including review of medications
• Following an established care plan, provide ongoing coaching and support to caregivers via app, including resource referral, and reinforcement of best practices.
• Develop trusting relationships with caregivers to influence change and reduce caregiver burnout
• Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards
• Other projects and duties as assigned.
• Perform other duties as assigned.

What You Will Bring

• Must hold an active Registered Nurse (RN) License in the state of employment
• Minimum two years of recent experience in direct care of elders or adults with disabilities
• Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required
• Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
• Proficiency in common software and web-based applications (e.g. MS Office, case management systems)
• Additional state-specific qualifications may be required.

The pay range for this position is $75,000 - $90.000. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
